We transitioned from an infant car seat to a Diono Radian R120 convertible car seat. After seeing my drowsy 15 month old's head slump drastically to the side, I searched for a side headrest. I tried sticking two rolled muslin blankets on each side of her head, but she kept pulling them down. The ears, however, don't bother her and she leaves them alone. Although these seem to be marketed mainly for infants, they work great on our toddler (for reference she's 21 lbs and 31"). I can see getting use out of them for at least a few more years. I'm sure the cotton fabric is fine, but I highly recommend the soft minky fabric.
